The TCM functions,
written from your own words.

A reviewer reads the note to see which case management functions you performed and why the client still needs the service. Each one appears only when your input describes it.

  1. 01

    Assessment

Each need and barrier you identified, and how it affects the client’s functioning or access to services.

  2. 02

    Planning

What you and the client or caregiver agreed to do next.

  3. 03

    Linkage, referral and coordination

The contacts, referrals and coordination you made, with their outcome as you reported it.

  4. 04

    Monitoring

Progress since the last contact and follow-through on earlier tasks. Left out when there is nothing to report.

Every note uses the goal from the client’s active service plan, closes with the continued need for case management, and lists one follow-up task for each need you mentioned.

What the note looks like

Fictional client, written to show the structure. Real notes follow what you enter, and you review every one.

Goal #1
“Client will attend all scheduled medical and psychiatric appointments to maintain stability in the community.”
Assessment
Caregiver reported the client missed one psychiatry appointment because of a lack of transportation, which puts medication continuity at risk.
Planning
TCM and the caregiver agreed to schedule Medicaid transportation for the next two appointments.
Linkage/Referral and Coordination
TCM contacted the transportation provider and confirmed a ride for the next appointment.
Continued need
The client continues to require TCM services to coordinate transportation and medical follow-up.
